PIP Forms
We can help complete PIP forms To help us to help you, before visiting the Advice Centre for help with these forms:
- For the PIP form you need to call 0800 917 2222 to request it. We cannot do this for you.
- Once you receive the form, read the form through so you understand what questions we need to ask.
- Gather any medical evidence you have and bring it with you.
- Bring a complete list of any prescription medication you are taking.
- Bring the names and details of any medical professional who are supporting you.
- Keep a diary of your daily routine and how your medical condition affects you.

UNIVERSAL CREDIT
Universal Credit has replaced the following benefits: Income Support, Income based JSA, Income based ESA, Housing Benefit, Child Tax Credit, Working Tax Credit.

Making an application for Universal Credit:. Apply for UC
NB: All correspondence will be through your Universal Credit journal. You must access your UC account regularly and ensure all actions are completed. Failure to do this may result in your money being stopped and potentially your account being closed.
If you cannot manage an online UC account, you will need to make a telephone claim. The UC claim number is 0800 328 5644
